Pay Rate: $33.00/hr $36.00/hr
Contract: 13 Weeks
Schedule: PM Shift 2:00 PM 10:00 PM Every Other Weekend
Guaranteed Hours: 36 hours/week
Experience: LTC experience preferred
EMR: Meditech
About the Role
We are seeking a Licensed Practical Nurse for a long-term care contract opportunity in New Lisbon WI. This role supports resident care medication administration condition monitoring family updates and collaboration with CNAs and nursing leadership.
Responsibilities

Administer medications while following the 5 Rights

Monitor residents and report changes in condition

Notify physicians family members and charge nurse as needed

Assist CNAs with call lights and resident support

Report any concerns related to abuse neglect or misappropriation

Maintain accurate documentation in Meditech

Perform other nursing duties as assigned
Requirements
-
Active Wisconsin LPN License required

Current CPR/BLS certification required
-
LTC or related nursing experience preferred

Strong attention to detail required

Must be able to work PM shift and every other weekend
