• Provide direct patient care including administering medications wound care and monitoring patient health.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Document patient progress and update medical records accurately and timely.
• Educate patients and their families on health maintenance and disease prevention.
• Adhere to all regulatory guidelines and maintain a safe and clean work environment.

• Valid Licensed Practical Nurse (LPN) license in the state of Michigan.
• Previous experience in a home care or clinical setting preferred.
• Strong communication and organizational skills.
• Ability to work independently and as part of a multidisciplinary team.
