Position Summary

The Licensed Practical Nurse (LPN) at the Southern Utah Veterans Home provides direct nursing care to veterans residing in the facility. The LPN works under the supervision of Registered Nurses (RNs) and other healthcare professionals to ensure high-quality compassionate care that meets the physical and emotional needs of our veteran residents.

Key Responsibilities
  • Provide routine nursing care including administering medications wound care and treatments as prescribed by physicians.
  • Monitor and document residents health status and report any changes or concerns to the supervising RN or physician.
  • Assist with activities of daily living (ADLs) such as bathing dressing and mobility support.
  • Collaborate with interdisciplinary team members to develop and implement individualized care plans.
  • Maintain accurate and timely medical records and documentation in compliance with facility policies and regulatory requirements.
  • Ensure a safe and clean environment for residents by following infection control and safety protocols.
  • Provide emotional support and education to residents and their families regarding health conditions and care plans.
  • Participate in ongoing training and professional development activities.
Qualifications
  • Current and valid Licensed Practical Nurse (LPN) license in the state of Utah.
  • Minimum of 1 year experience in a long-term care skilled nursing or similar healthcare setting preferred.
  • Basic Life Support (BLS) certification required.
  • Strong communication and interpersonal skills.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • Compassionate and patient-centered approach to care.
  • Familiarity with electronic health records (EHR) systems is a plus.
Work Environment

The Southern Utah Veterans Home is dedicated to providing a supportive and respectful environment for both residents and staff. The LPN will work primarily in a long-term care facility setting interacting closely with veterans and healthcare team members.
