Position Summary

The Licensed Practical Nurse (LPN) is responsible for assessing residents administering medications and reporting any health changes to the physician and charge nurse. This role also involves updating family members and collaborating closely with Certified Nursing Assistants (CNAs) to ensure high-quality patient care.

Essential Job Duties

• Medication Administration:Safely administer medications to residents following the 5 Rights of medication administration.
• Patient Oversight:Monitor residents and report any changes in condition promptly to the charge nurse.
• Communication:Update physicians family members and appropriate staff regarding changes in a residents condition as needed.
• Team Collaboration:Support and assist CNAs with answering call lights and completing other floor responsibilities.
• Compliance & Safety:Report any allegations of abuse neglect or misappropriation directly to the Director of Nursing and Administrator.

Required Qualifications

• Licensure:Must hold and maintain an active unrestricted Wisconsin (WI) LPN license.
• Certifications:Current CPR / BLS certification is required.
• Experience:1 years of related work experience in a Long Term Care (LTC) setting is preferred.
• Skills:Exceptional accuracy strong clinical judgment and close attention to detail.
