At Auburn Crest Primary & Palliative Care we bring medical care directly to seniors in the assisted living and memory care communities they call home. Were looking for a compassionate Licensed Practical Nurse to be the steady trusted connection between our patients their families community staff and our Nurse Practitioners across Ada & Canyon County.

What Youll Do
- Serve as the go-to clinical contact for the assisted living and memory care communities in your county
- Partner with our RNs and Nurse Practitioners to schedule and prioritize patient visits
- Process referrals medication requests and refills as directed by providers
- Coordinate labs durable medical equipment (DME) and other clinical resources
- Send track and follow up on provider orders
- Educate patients families and caregivers so everyone feels confident in the care plan
- Keep accurate timely documentation in our electronic medical record
What You Bring
- Active LPN license in Idaho
- 1 year of nursing experience preferred
- Experience in geriatrics assisted living memory care palliative care or hospice is a plus
- Current CPR/BLS certification
- Strong organization and clear communication both verbal and written
- Valid drivers license auto insurance and reliable transportation for local travel
