Position Summary

Licensed Practical Nurse II isaccountable for the delivery of patient care as delegated by and under thedirection of a physician or registered nurse (RN) utilizing the Nursing Processand Relationship-Based Care model as the framework. The Licensed PracticalNurse II provides direct patient care and takes initiative to improve patientoutcomes by integrating Shared Leadership Evidence Based PracticeRelationship Based Care and standards of nursing excellence into clinicalpractice. Through therapeutic relationships with patients families self andcolleagues the Licensed Practical Nurse II functions as an interdisciplinaryteam member who provides quality patient care and fulfills unit andorganizational goals.

Duties & Responsibilities

• Carries out or assists with diagnosticprocedures which may include identifying and recording subjective symptomssuch as pain; observing and recording objective symptoms such as body excretaand discharge pulse temperature respiration blood pressure and changes incondition of body; collecting specimens for the laboratory tests such astesting urine for blood sugar acetone and specific gravity.
• Carries out therapeutic procedures whichmay include applying binders and bandages and applying heat or cold packs.
• In accordance with established nursingpolicies and procedures administers oral medications and intra muscularsubcutaneous and intra dermal injections. Performs intravenous therapy asdirected by an RN or physician.
• Communicate with patients to assessemotional and mental well-being; identifies and reports patients requiringspecial assistance.
• Charts and reports nursing careactivities performed and reports observations made.
• Performs audits of narcotics by countingsupply on hand comparing amount against record of use and reportingdiscrepancies.
• Dispense medication (including methadone)in accordance with protocols set by IDPH DBAR DEA and FDA.
• Monitor the appropriateness of methadoneand other medication dosages.
• Observe and document clients physicalcondition including signs of intoxication or withdrawal and consult withmedical staff as needed.
• Conduct initial medical assessments andmaintain accurate documentation in client records.
• Follow up on TB tests reactive resultsand any referrals or recommendations from medical visits.
• Administer breathalyzer tests as requiredto determine recent substance use.
• Maintain accurate medication recordsincluding DBAR reports methadone counts and provider orders.
• Ensure proper storage and safekeeping ofcontrolled substances and prescription medications.
• Collaborate with medical providers andcounselors to support integrated care planning.
• Participate in daily clinical rounds utilization reviews and qualityassurance initiatives.
• Complete thorough and timely nursingnotes and documentation for all patient interactions.
• Performsupportive tasksrelatedto nursing care which may beadjusted or reassignedbased onprogram or patient needs. Maintainprofessional and ethical standardsinall interactions with clients and staff.
• Stay informed of updates insubstanceuse treatment protocolsandrelevant nursing practices. Mustwork effectively with diverse individuals and organizations across culturalgenerational ethnic and professional backgrounds.
• Performs other related duties asassigned.

Minimum Qualifications

1. Possession of a valid license as a Licensed Practical Nurse issued by the Illinois Department of Financial and Professional Regulation Division of Professional Regulation.
2. Six months of nursing experience in a health care facility (such as a hospital nursing home medical clinic prison mental health facility or closely related facilities).
3. Current AHA BLS/CPR and other required unit specific certifications
4. Effective communication skills (oral and written) that facilitate therapeutic relationships between patients family and health care providers
5. Intermediate computer skills with the ability to become proficient in the Electronic Health Record (EHR) with on-the-job training
6. Excellent attendance interpersonal skills and work history required.

Preferred Qualifications
One year LPN experience preferred. Demonstrated knowledge of substance use disorders detoxification and recovery processes. Understanding of general nursing practices and medical terminology. Working knowledge of Illinois state and federal regulations applicable to behavioral health and medication-assisted treatment (MAT). Strong written and verbal communication skills must be able to utilize a computer phone etc. for extended period of time and write or type notes for at least two (2) to four (4) hours per day.
