Licensed Practical Nurse Behavioral Health Home Visits
Full-Time Worcester Area

We are seeking a compassionate Behavioral Health Licensed Practical Nurse to join our supportive Worcester-area this role you will provide personalized nursing care in patients homes under the direction of a Registered Nurse or physician. You will build meaningful relationships while helping patients manage their conditions medications and daily care needs.

What Youll Do

As a Behavioral Health LPN you will:

  • Review the patients Plan of Care before each visit and provide care according to RN delegation physician orders assessment findings and established clinical standards

  • Deliver skilled nursing services requiring specialized nursing knowledge under the direction of a Registered Nurse or physician

  • Incorporate established patient goals into each nursing visit and document the care and progress accurately

  • Monitor each patients physical behavioral emotional and environmental condition

  • Promptly communicate significant findings safety concerns or changes in condition to the RN Case Manager physician Clinical Supervisor and other members of the care team

  • Report unsafe conditions or concerns within the patients home environment to the appropriate clinical personnel

  • Support medication management by reviewing and documenting medication regimens and updating medication profiles according to established procedures

  • Educate patients and caregivers on appropriate self-care techniques medications symptom management and safety practices

  • Help patients and caregivers recognize warning signs and understand when to contact the clinical team

  • Implement patient-safety interventions using available family caregiver and community resources

  • Recommend updates to the Plan of Care when new needs or concerns are identified

  • Collaborate with the RN Case Manager for clinical guidance support and instructions

  • Take appropriate action to help prevent avoidable hospitalizations and emergency room visits

  • Communicate professionally and compassionately with patients families caregivers physicians and agency personnel

  • Treat patients caregivers and their personal property with dignity respect and care

  • Maintain accurate timely and complete clinical documentation

  • Protect patient and employee privacy and confidentiality in accordance with HIPAA requirements

  • Follow Elara Carings mission policies procedures and quality-of-care standards

  • Perform additional duties and special projects as assigned

What Youll Need
  • High school diploma or GED

  • Graduate of an accredited practical nursing program

  • Current unrestricted Licensed Practical Nurse license in Massachusetts

  • At least one year of nursing experience in an acute-care or equivalent clinical setting

  • Behavioral health or home health experience preferred

  • Current CPR certification through the American Heart Association or American Red Cross

  • Strong communication organization and clinical documentation skills

  • Compassionate dependable and dedicated approach to patient care

  • Ability to work independently while collaborating with an interdisciplinary care team

  • Reliable transportation a valid drivers license and current automobile insurance

  • Willingness and ability to travel throughout the assigned Worcester-area territory

  • Sit stand bend lift and move intermittently and be able to lift 50100 lbs.
