Summary of Job Duties:
The Transplant Licensed Social Worker is a member of the solid organ transplant multidisciplinary care management team and responsible for assisting in the patient management in all phases of care.. The role reflects appropriate knowledge of Social Work scope of practice current state requirements CMS Conditions of Participation EMTALA Patient Bill of Rights AB 1203 and other Federal or State regulatory agency requirements specific to the Social Work role. The Social Work team member will partner with the multi-disciplinary team utilizing best practices relevant data and compliance with the mission/philosophy standards goals and core values of UAMS: Comfort Hope and Healing. Primary elements of the social work role include: assessment of the psychological financial and discharge planning needs of the patients and families providing information support counseling and referrals to other levels of care and appropriate resources for the transplant patient.

Qualifications:

Minimum Qualifications:

  • Masters degree in Social Work

  • Must be a licensed by an Arkansas Professional Board as a Licensed Masters Social Worker (LMSW) by date of hire

Preferred Qualifications

  • Experience in Case Management

  • Experience working in a medical setting

  • Certified Case Manager (CCM) or American Case Management (ACM) certification

  • Experience working with the Transplant Patient Population


Additional Information:

Primary elements of the social work role include:

Assessment of the psychological financial and discharge planning needs of the patients and families providing information support counseling and referrals to other levels of care and appropriate resources for a variety of age-specific populations including neonatal pediatric adolescents adults and geriatric.
