Position Title: Lead Direct Support Professional - White Bear Lake Maplewood Roseville Woodbury Little CanadaDepartment:ProgramGrade Range/Job Status:Full-time Non-Exempt-HourlyReporting Relationship:Program ManagerSupervisory Responsibilities:Direct Support ProfessionalTypical Schedule:Must be willing and be able to work flexible hours/days including evenings overnights weekends and SummaryThe Lead Direct Support Professional will assist the Program Manager with administrative tasks in addition to providing daily support to people with intellectual and/or developmental disabilities living inHammer & NER homes and apartments. Lead Direct Support professionals develop meaningful relationships and connections with individuals providing them with a safe living environment and promoting life skills. All employees are expected to center model and champion Hammer & NERs core values: Person-Centered Relational Opportunistic and Duties and SupportPromptly schedule medical appointments for residents served in the home or apartment programPromote communication and active lifestyle for residents by scheduling activities and events in the communityTrain Direct Support Professionals in the following areas:oDocumentation in CarasolvaoEmergency proceduresoProgram specific duties including use of medical equipment appliances and financesMedication administrationoDocumentation through CarasolvaoAuditing medication cabinetoOrder medicationsoWorks closely with nurse and pharmacy staff to ensure adequate medical supply and orders medication supplies as Living SupportsMaintain a clean environment in homes and apartments by following all cleaning and sanitizing rules posted in the apartment or homeAssist residents with all daily living skills which may include:oBathing showering toiletingoMeal planningoMoney managementAdminister medicationsAssist residents to any scheduled medical respond to communications through outlook Teams and SharePointCheck program communications through SharePoint dailyCollaborates with Program Manager to resolve performance issues regarding Direct Support addresses any complaints and under the direction of the Program Manager follows procedures to resolveImmediately report all accidents injuries or illnesses involving residents to the appropriate party (nurse Program Manager Program Emergency phone Program Director or 911) and ComplianceMust be effective in monitoring all assigned trainings are complete by due dateMust be diligent in maintaining confidentiality and privacyby following HIPPA adhere to the laws as a mandated reporter to protect vulnerable adults from abuse neglect and exploitationAttend all meetings scheduled by Program Directors or ManagersMust meet the needs of each resident served in our programs to sustain quality of life and optimal careEssential Knowledge and QualificationsMust be 18 years of age or in using computers and/or smart demands include:oAbility to lift 40lbs (such as lifting a wheelchair into a van) pushing or pulling an individual in a wheelchair or while assisting with personal caresoHousehold chores and physical cares may include twisting/turning kneeling/squatting reaching and assisting with pivot specialized medical or adaptive equipment including use of lifting equipment and driving a lift interpersonal skills and ability to communicate effectively throughout the organization;must be fluent reading and speaking to remain positive and foster a positive work environment while facing pressure or demands from multiple be cleared by a background check through the MN Department of Human a valid drivers license and a driving record that meets insurance requirements; ability to drive an organizational ability and commitment to working with individuals and teams that are mixed across lines of difference such as race gender-identity sexual orientation religion ability age class and immigrant status.#IND
