• Starting Base Pay Range:$33.06 - $36.78 per hour PLUS 3%
• Availability Pay for providing youth & families coverage after hours:$75.00 per pay period (2 times per month)

The Lead FBH Clinical Care Coordinator (FCCC) will coach Care Coordinators in their work work directly with children and youth diagnosed with severe and chronic behavioral health conditions in coordinating care. This will include coaching others in the development of a Child and Family Team (CFT) and facilitation of CFT meetings and coaching around implementing and monitoring the Cross-System Care Plan. The Lead Clinical Care Coordinator coaches Care Coordinators to provide support for the team planning process and to coordinate with the Clinician who works with the child and their family to design a behavioral health Treatment Plan comprised of treatment strategies based upon family needs strengths and goals.

The Lead Clinical Care Coordinator provides coaching to Care Coordinators in one-on-one crisis response and support to high-risk children/youth and their families in the home and community when needed. Clinical response and support include the planning and provision of supervision for safety assistance in accessing and participating in community activities and mentoring and coaching positive behavioral strategiesalong with facilitating child and family teams. The Lead Clinical Care Coordinator partners with Clinical Supervisors to ensure Care Coordinators have the skill to ensure a coordinated Cross-System Care Plan is in place andthat the Care Coordinator works to ensure the success of that plan using team-based approachesand a coordinated child-family team planning position requires capacity for flexible 24/7 on-call availability to support Care Coordinators with rotating on call responsibilities with other team members.

JOB SUMMARY:

• The Lead Clinical Care Coordinator in partnership with the Clinical Supervisor will coach Care Coordinators to document all Coach Clinical Care Coordinators to complete family and behavioral health assessments for all children/youth including completion of the CANS (Child Adolescent Needs Strengths) working directly with the youth and family and their supports with the goal of multi-dimensional assessment in all life areas including home school and community.
• To enhance motivation and participation in services the Lead Clinical Care Coordinator will coach Care Coordinators to participate in a comprehensive strengths assessment or Strength Need and Culture Discovery that includes the identification of family strengths; interests talents aptitudes as well as cultural capacities. The Care Coordinator will ensure that these are shared with the team to consider strengths application and implementation throughout the intervention.
• Based on child/youth treatment needs and mental health diagnosis the Lead Clinical Care Coordinator will coach Care Coordinators to collaborate with assigned Clinician and their WISe team to identify and support consumer access to behavioral health and substance abuse treatment services available in the community. The Lead Clinical Care Coordinator will coach Care Coordinators to assist with physical health care needs for the child/youth as well.
• The Lead Clinical Care Coordinator will coach Care Coordinators to facilitate a Child and Family Team (CFT) planning process that includes at least monthly Child and Family Team meetings for enrolled youth incorporating strengths needs preferences and strategies across multiple life areas
• The Lead Clinical Care Coordinator will coach the Care Coordinator to work with the WISe team to support implementation of prescribed behavioral strategies and treatment services to enrolled children and youth modeling and teaching social behavior self-protection skills and awareness and developmentally appropriate coping and social interaction skills.
• The Lead Clinical Care Coordinator will coach Care Coordinators in their role as part of a clinical team to assess family need for community resources to ensure safety and recovery-oriented treatment and to work with other Medicaid funded providers and other supports to introduce and coordinate youth and family with needed behavioral health and community resources.
• The Lead Clinical Care Coordinator will coach Care Coordinators to support safety by acting as a First Responder helping them learn the skills to provide direct crisis response de-escalation emergency supports for children in crisis.
• The Lead Clinical Care Coordinator will coach Care Coordinators to meet with youth and family in their homes assessing home environment which may include: performing safety sweeps in the home (or foster home) for children and youth with suicidal ideation and other high-risk safety concerns (as identified in a Mental Health assessment or crisis referral).
• The Lead Clinical Care Coordinator will support Care Coordinators in their ability to demonstrate appropriate judgement by reporting information related to safety or therapeutic progress immediately as needed to the appropriate entity (DCYF intake Law Enforcement) Clinical Supervisor and the child/youths treatment team.
• The Lead Clinical Care Coordinator will coach Care Coordinators in their ability to demonstrate an understanding of behavioral health services and their role in wellness and recovery by communicating role responsibility and agency philosophy to assure mutual respect confidence and trust with the child family and other stakeholders.
• The Lead Clinical Care Coordinator will assist Care Coordinators in their ability to demonstrate clinical engagement and acceptance of the child youth and their family meeting and engaging the family communicating compassion support respect and enthusiasm for their recovery journey.

WHAT WE ARE SEEKING IN A CANDIDATE:

• Bachelors Degree in social sciences social services mental health field or equivalent field. (This qualification is based on new contract expectations).
• Two or more years of experience with WISe Care Coordination with relevant experience with children in crisis.
• Age 21 or older per DSHS requirements.
• Eligible to become an Agency Affiliated Counselor with WA State (HIV/Aids required).
• Proof of negative TB test within past 12 months.
• Demonstrate effective verbal and written communication.
• Demonstrate maturity professionalism and initiative.
• Availability to work flexible hours (evenings nights weekends).
• Ability to make visualsafety and well-beingassessments of family home conditions and crisis situations.
• Must have reliable transportation valid drivers licenseandautomobile insurance.
• Must have an acceptable drivingrecord per CCS driving policy(required to provide current driving abstract).
• Applicantmust successfully pass required background clearances prior to an offer of employment.
