Job Summary

ESSENTIAL FUNCTIONS

  • Provides regional (site-specific) non-patient care leadership as a resource toAPPsin addressing employee relationsissues that may arise.
  • Serves as a resource in the development ofHOPCoValue-BasedCarerelationships.
  • Works collaboratively with other LeadAPPsandVP of Value-Based Careto effectively recruit and onboard newAPPs
  • Provides analysis of quality metrics.
  • Conducts performance evaluationsforAPPs
  • Assistsin creating work schedulesfor APPs.
  • ProvidesAPPtraining and education
  • Assesses patient health status including state of wellness compliance withthecare plan anddeterminesappropriate diagnosticand therapeutic procedures.
  • Provides acute and chronic patient care including taking historiesconductingphysicalsmonitoringtherapies and medications.
  • Triages patient calls and evaluates patient problems.
  • Triagesemergencies
  • Documents patient information and care provided inpatient recordsandmaintainspatient statistics.
  • Educatethepatients and families asappropriate. Provides continuity of care.
  • Consults with otherstaffas necessary.
  • Maintains patient confidentiality.
  • The job holder mustdemonstratecurrent competencies forthejob position.
  • Other duties as assigned.

EDUCATION

  • BA/BS MS preferred. Graduate of AMA accredited physician assistantor nurse practitionerprogram.

EXPERIENCE

  • Experience in orthopedics is required.

REQUIREMENTS

  • Currentstate APPlicense certified byNationalCommission on Certification of Physician Assistantsor Accreditation Board of Specialty Nursing Certification (ABSNC) current CPR certification.
  • At least one year of experienceinanAPP leadership role.

KNOWLEDGE

  • Knowledge of acute and chronic protocols administration of medicationsdeliveringphysical care followingthetreatment plan.
  • Knowledge of how to assess patient health and measure/recordphysiologicaland growth indices.
  • Knowledge of health promotion principles and techniques and patient learning measurements.
  • Knowledge of quality metrics and standards.

SKILLS

  • Skill in gathering and analyzing physiological socioeconomicand emotional patient data.
  • Skill in accurately evaluating patient problems in person or via phone and providingappropriate advice interventionor referral
  • Skill in evaluating and revising care plansbased on patient changes.

ABILITIES

  • Ability to make responsible decisions within the scope ofthedependent medical practitioner.
  • Ability to develop health education programs and materials.
  • Ability to work collaboratively with all members of the health care team.
  • Ability to analyze and respond to data.

ENVIRONMENTAL WORKING CONDITIONS

  • Normal office environment.
  • HIPAA compliantenvironment ifremote.
  • Some travel within the community.

PHYSICAL/MENTAL DEMANDS

  • Requires sitting and standing associated with a normal office and remoteenvironment.
  • Some bending and stretching arerequired.
  • Manual dexterity using a calculator and computer keyboard.
