We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) Licensed Professional Counselor (LPC) or Licensed Marriage & Family Therapist (LMFT) to join our team at our New Roads Behavioral Health Clinic and Pointe Coupee School-Based Clinic.

This is an excellent opportunity to make a meaningful impact in a collaborative patient-centered healthcare environment serving children adolescents and families.

Responsibilities Include:
Assessing implementing and monitoring behavioral health treatment plans
Conducting mental health evaluations and obtaining patient histories
Providing counseling education and support for patients and families managing conditions such as ADHD anxiety PTSD and related behavioral health concerns
Collaborating with physicians and healthcare teams to ensure coordinated care
Performing behavioral health screenings using evidence-based assessment tools
Supporting Patient-Centered Medical Home (PCMH) initiatives including motivational interviewing and self-management coaching
Assisting patients in navigating healthcare and community resources
Maintaining accurate and compliant patient documentation
