Compensation:

• Base salary: $155000

Job/Practice Description: Inpatient Palliative Care NP

• Full-time inpatient position: 75% at one hospital in Findlay OH and 25% at a second hospital in Lima OH
• MondayFriday schedule 8:00 AM4:30/5:00 PM
• Shared evening and weekend telephone call rotation
• Perform inpatient palliative care consultations and follow-up visits
• Manage symptoms for patients with serious chronic and life-limiting illnesses
• Facilitate goals-of-care discussions and advance care planning with patients and families
• Assist with discharge planning including transitions to home-based palliative care or hospice
• Provide inpatient rounding for hospice General Inpatient (GIP) patients at the second hospital as needed
• Professionals with a passion for palliative care encouraged to apply

Requirements:

• Certified Nurse Practitioner (Acute Care NP preferred; palliative care or hospice experience welcomed)
• No experience needed!
• Strong communication interpersonal and organizational skills
• Comfortable navigating complex conversations related to serious illness prognosis and end-of-life planning
• Ability to work independently while collaborating within an interdisciplinary team
