We reach more than 65000 individuals and families each year at our 90 locations including 10000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are in living in poverty reflecting the challenges faced by nearly 1.7 million New Yorkers today.
HomeBase provides case management and essential services to at-risk individuals and families to prevent homelessness. Individuals and families receive services that will help overcome problems with public benefits housing education health care and other issues that could impede their ability to maintain stable households.
Position: IntakeSpecialist
Reports To: Assistant Director/Program Manager
Location: Fulton Street Brooklyn NY
What The Intake Specialist Does:
- Coordinate the intake process with individuals and families facing housing crises. This includesreviewingeligibilitydocumentation gathering detailed information about their current situation housing history income expenses barriers to housing stability and support systems.
- Assess the level of homelessness risk based on established criteria and program guidelines.
- Effectively communicatethe homelessness prevention programs services eligibility requirements and limitations to potential clients.
- Schedule appointments make referrals and provide all individuals with information on community resources.
- Ensure that all client resources are up-to-date and available inthe receptionarea.
- Perform periodic outreach activities to publicize the availability of services.
- Perform data entry filing copying and faxing of documents.
- Provide referrals to appropriate internal and external resources such as emergency shelters housing assistance programs legalservices employment services mental health services and community-based organizations.
- Collaborate effectively with program staff including Program Supervisors case managers case support workers and outreach workers to ensure coordinated service delivery.
- Stay informed about current housing trends local resources and relevantguidelinesrelated to homelessness prevention.
- Participate in regular team meetings supervision sessions and training opportunities to enhance skills and knowledge.
- Adhere to agency policies and procedures ethical guidelines and professional standards of conduct.
- Report to work daily with fullychargedcell phone for multifactor authentication.
- Conduct home visits and inspections for any open HomeBase case; not limited to assigned cases only. Evaluate living conditions and/or conduct inspections as per DHS/DSS/HRA standards.
- Create and maintain client files both hard copy and electronic.
- Assist with reception duties when necessary.
- Other duties as assigned.
What are theQualifications for the Position:
Education Licenses and/or Certifications Required:
- High school diploma or G.E.D.
Minimum Experiences and Skills Required:
- Fouryears of relevant experience preferably in a mid-sized (250 employees) organization.
- ProficiencyinMicrosoft Office (Excel Word Outlook)
- Strongwriting skills with the ability to draft assessments service plans progress notes weekly reports and general client-related correspondence.
- Goodcommunication skills with the ability to effectively present information and respond to questions from supervisors clients funders other organizations and thegeneral public.
- Ability to apply basic mathematical concepts such as fractions decimals and percentages to practical situations.
- Goodproblem-solving skills with the ability to define problems collect data establish facts and draw valid conclusions.
- Willingness to learn and adapt
Preferred Qualifications:
- Proficiency in a language other than English (e.g. Haitian/Creole French Spanish Russian etc.) preferred not required.
- Familiarity with HRA benefits NYC housing programs and community resources preferred.
Compensation: $42000.00 - $44000.00 annually
Status: Full-time (35 hours per week)
