Overview:

This position is eligible for up to a $20000 sign-on bonus for qualified candidates.

Status: Full-Timepermanent position (40 hours)

Standard working hours: 8:00 AM 4:30 PM EST MondayFriday with rotating weekends (Saturday or Sunday) every 6 weeks. A weekday off is provided when working a weekend day so hours do not exceed 40 per week.

Location: This is an onsite position at Sentara Princess Anne Hospital 2025 Glenn Mitchell Drive Virginia Beach VA 23456.

Job responsibilities:

• The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management services and quality outcomes for patients across the healthcare continuum primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS.
• Directly support those in hospital patients identified in need of comprehensive case management.
• Promotes effective utilization and monitoring of health services collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions.
• Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical mental and psychosocial needs of the inpatient/caregiver.
• Functions in one of the following practice settings: Hospitals Only

Education:

• Bachelors Degree in Nursing preferred

Certification/Licensure

• Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED
• BLS within 90 days of hire.
• Case Management (ACM CCM CCCTM CMAC or CMGT-BC) certification is required within one year of eligibility.

Experience:

• 1 year of nursing experience REQUIRED
• 1 year of acute nursing experience or care case management experience preferred

We provide market-competitive compensation packages inclusive of base pay incentives and benefits. The base pay range for full-time employment is $89544.00 - $136468.80 annually. Additional compensation may be available for this role such as shift differentials on-call pay shift premiums extra-shift incentives or bonus opportunities. Compensation within the range may vary based on qualifications experience location market conditions and business needs. Note: If an annual salary is posted it is based on a full-time colleague working 2080 hours annually.
