Job Summary
Position Title: Incident Coordinator
Location: Remote OH Candidates can reside anywhere but must have knowledge of Ohios MyCare Waiver with OH LICENSE
Duration: 6 months (possibility to extend or convert to FTE)
Schedule:
- Training: Mon Fri 8am 5pm EST (approx. 4 weeks no time off permitted during training)
- Regular: Mon Fri 8am 5pm EST
- No weekends or holidays; overtime only if required
PR: $28 to $30/hr.
Summary:
This team reviews the incidents and works with the care coordination teams to create person-centered prevention plans to help ensure the health safety and welfare of all our members.
Job Responsibilities:
- The Incident coordinator is responsible for monitoring the incident mailbox and triaging any incidents that come in and forwarding any emails related to open cases.
- They will be responsible for reviewing the Critical Incident Report daily and checking for previous incidents.
- Assigning the cases appropriately and placing the information on the internal tracking sheet.
- The coordinator will be responsible for following all MyCare Waiver and MyCare SRS cases and uploading all documents into the IMS system.
- Staff performance is measured by timeliness of submission timeliness of submitting Prevention Plans and closing Reportable cases; as well as accuracy of incident submission and PPs.
- This role is unique to Ohio. Only Ohio tracks incidents in conjunction with the state.

Job Requirements:
- Education: Degree and or certificate/licensure required in a field of Nursing Social Work and/or Counseling
- Licensure: Must have an OHIO licensure as an LPN SWA CHW LSW or LPC.
- Experience: 2 years experience managed care; must have knowledge of Ohios MyCare Waiver & OH incident reporting. Nursing Social Work and/or Counseling.
Skills:
- Working with a computer (TECH SAVVY) and ability to move seamlessly from program to program
- Critical thinking skills a must
- Use of Excel Word Outlook teams
