Responsibilities

• Accept and treat patients within your designated geographic area

• Evaluate patients needs for physical therapy in the home setting and develop individualized treatment plans

• Develop a plan of care and treatment goals in consultation with the physician patient and family

• Plan prepare and implement customized therapy programs to restore function and promote independence

• Instruct family members PTAs Home Health Aides and caregivers on appropriate therapy techniques

• Provide timely and accurate documentation for each visit

• Communicate effectively with patients families and interdisciplinary team members especially around discharge planning

• Deliver care in compliance with agency policies and professional practice standards

Qualifications :

• Graduate of an accredited Physical Therapy program

• Licensed Physical Therapist in the state of New York or eligible for licensure

• Home health experience is preferred but not required we will provide you with a mentor to help with documentation and compliance

• Ability to travel to patients homes within the assigned geographic territory

Additional Information :

The anticipated base salary range for this position is $95000$110000 annually. Salary is based on various factors including relevant experience knowledge skills other job-related qualifications and geography.

This position is eligible for a $3000 sign-on bonus. Additional terms and conditions may apply.
