Deaconess Illinois is seeking a Hospital Pharmacist to provide safe effective and patient-centered pharmaceutical this role youll work collaboratively with physicians nurses pharmacy technicians and other healthcare professionals to optimize medication therapy and support positive patient outcomes.

As a Hospital Pharmacist you will help coordinate the day-to-day delivery of comprehensive pharmaceutical care while ensuring compliance with professional regulatory and accreditation standards.

Key responsibilities include:

• Review and verify medication orders for accuracy safety and appropriateness
• Perform computerized order entry and prospective medication review
• Oversee medication preparation and distribution activities
• Support IV admixture preparation and other medication distribution functions
• Evaluate medication therapies and make recommendations to optimize patient outcomes
• Communicate with physicians nurses and other healthcare professionals regarding medication orders and therapy recommendations
• Document clinical interventions and outcomes as appropriate
• Provide professional oversight and direction to pharmacy technicians
• Participate in educational activities to maintain and advance clinical pharmacy knowledge
• Promote medication safety and adherence to established pharmacy policies and procedures

Education

• Pharmacy degree (B.S. Pharmacy Pharm.D. or equivalent) from an accredited college of pharmacy required

Experience

• One year or more of experience in a hospital or health system setting preferred

Licensure

• Current license as a Registered Pharmacist in the State of Illinois required

Hourly Pay Range: $54.29$81.44

This position may be eligible for evening night and weekend shift incentives based on company policies and applicable job codes.

Eligible team members may also qualify for a performance-based bonus based on established eligibility criteria and performance goals. Additional details regarding incentives and bonus opportunities will be provided during the interview process or upon hire.
