With a staff of 950 and an annual budget of $140 million Project Renewal is one of the larger social service nonprofits in New York : Housing Case Manager Salary Range: $38000 - $46000 per yearProgram: Multiple opportunities are available in the Bronx Brooklyn and Manhattan in our Housing Programs Bedford Greenhouse I and II (Bronx)The Villa (Bronx)Parole Support and Treatment Program (Scattered Sites)In-Homes Now & PRI Transitions (Scattered Sites)Geffner House (Mid-town Manhattan)Renewal House (Bronx)Fletcher Residence (Bronx)Clinton Residence (Mid-town Manhattan)Leona Blanche House (Bronx)St. Nicholas House (Upper Manhattan)Essential Duties and Responsibilities:The essential duties of the Housing Case Manager include but are not limited to the following activities:Provides intake psychosocial and substance abuse assessments to and develop an appropriate treatment plan for each client in the assigned case management services to assist in the creation of and adherence to individual Service Plans identifying short-term and long-term goals and objectives to be home visits coordinates medical psychiatric and substance abuse servicesMonitors medication compliance as necessaryMaintains written information or case notes regarding all contact and intervention with the clients in AWARDS/CARES systemConducts individual and group supportive counseling sessionsOrganizes and educates tenants through groups and other activities presenting issues of interest to the clients such as money management employment opportunities clients to community servicesParticipates in weekly clinical meetings to discuss high risk clients; updates spreadsheets related to clients progressPerforms other duties as assigned by supervisory :Bachelors Degree in Social Work or related field of experience required 1-2 years of experience working with homeless mentally ill adults or other special needs population preferred Excellent oral writing and listening skills Proficiency in computer required.
