Our Hospice Nurse Care Manager (RN) Will:
- Provide individualized care by treating each patient as they would want a loved one treated.
- Develop care plans that reflect each person and their needs.
- Be detail-oriented knowing that carefully managed patients (medications treatments communication aide supervision etc.) have more comfort and dignity.
- Focus on customer service when interacting with patients families communities/facilities and all partners and co-workers.
- Help team members improve through instruction and support.
- Build and manage meaningful relationships with all partners.
Our Hospice Nurse Care Manager (RN) Has:
- An active state-required RN license or a compact state license.
- 2 years of hospice experience.
- On-call/triage experience.
- An active Drivers License with a reliable vehicle insurance and an acceptable driving record.
- The ability to meet the physical requirements for patient care with reasonable accommodations (e.g. Independently lift at least 30lbs).
- Humility to know that even experts learn something new every single day.
- A positive attitude and desire to make every day better for the people they encounter.
