Were hiring a Hospice Case Manager (RN) who wants a predictable weekday schedule meaningful patient relationships and the autonomy to provide compassionate care in patients homes.

Youll step into a field-based hospice role providing case management throughout Gwinnett County. Youll conduct assessments develop care plans manage symptoms educate patients and families and coordinate care while working alongside a supportive interdisciplinary hospice team.

You hold an active Georgia RN license
You maintain current CPR certification
You have at least 1 year of nursing experience
Youre comfortable providing independent compassionate care in patients homes
You have a valid drivers license and reliable insured transportation

Compensation & Benefits

Base salary: $90000$95000
Mileage reimbursement
Cell phone stipend: $60 per month
Health insurance: Employer pays 75%
Dental insurance: $7.48 per pay period
Vision insurance: $1.37 per pay period
