Looking for a nursing role where you can use your clinical judgment build relationships and manage your day Join Elara Caring as a Home Health RN Case Manager serving patients throughout Boston Proper including Chinatown.

Youll provide skilled care in patients homes and help them recover manage their health and stay independent. And because parking is part of working in Boston we provide a paid parking pass for this territory.

Youll guide patients through their home health care from assessment to discharge. That includes providing skilled nursing visits developing and updating care plans managing an assigned caseload and coordinating with physicians and other clinicians. Youll also teach patients and caregivers how to manage medications treatments and health conditions at home.

• Current unrestricted Massachusetts RN license
• At least one year of clinical nursing experience
• Strong assessment communication and time management skills
• Ability to work independently and travel throughout Boston Proper including Chinatown
• Valid drivers license current auto insurance and reliable transportation
• Ability to sit stand bend lift and move intermittently and lift 50100 pounds
