As a Registered Nurse youll play a vital role in helping patients recover manage chronic conditions and remain safely at home.

Responsibilities include:

• Performing comprehensive nursing assessments in patients homes.

• Developing implementing and updating individualized plans of care.

• Providing skilled nursing care treatments and patient education.

• Collaborating with physicians therapists and interdisciplinary team members to coordinate care.

• Monitoring patient progress and communicating changes in condition.

• Educating patients and caregivers to promote confidence and independence.

• Maintaining timely and accurate clinical documentation.

• Delivering care in accordance with regulatory and organizational standards.

• Current unrestricted Texas Registered Nurse (RN) license

• Minimum one (1) year of nursing experience

• Strong assessment and critical thinking skills

• Valid drivers license reliable transportation and current auto insurance

• Ability to travel throughout the assigned service area

• Physical Requirements: Sit stand bend lift and move intermittently throughout the workday. Must be able to lift push pull and support 50100 pounds including assisting with patient transfers and moving equipment.

• Home Health or Hospice experience

• OASIS documentation experience

• Homecare Homebase (HCHB) experience

• Associate or Bachelors Degree in Nursing
