Job Summary:

We are seeking a compassionate and skilled Home Health Registered Nurse RN to join our team of dedicated healthcare professionals.

Responsibilities:

• Provide patient-centered care to patients in their homes focusing on their unique needs and goals

• Conduct initial assessments develop and implement individualized care plans and evaluate patient progress

• Collaborate with interdisciplinary teams including physicians therapists and caregivers to ensure comprehensive care

• Administer medications treatments and therapies as prescribed by physicians and in accordance with agency policies

• Document patient information accurately and in a timely manner ensuring compliance with regulatory requirements

• Participate in quality improvement initiatives to enhance patient outcomes and agency performance

Requirements:

• Current RN licensure in the state of California

• Minimum 1 year of experience in home health or a related field

• Strong communication and interpersonal skills with the ability to work effectively with patients families and interdisciplinary teams

• Ability to lift move and transfer patients as needed

• Reliable transportation and the ability to travel to patients homes
