• Deliver skilled nursing care to patients in their homes while promoting safety independence and quality of life.

• Complete comprehensive nursing assessments and develop individualized plans of care.

• Perform admission routine and follow-up visits based on physician orders and patient needs.

• Monitor patient progress and identify changes in condition communicating findings promptly with physicians and the interdisciplinary care team.

• Educate patients and caregivers on disease management medications treatments and self-care.

• Coordinate care with therapists aides physicians and other healthcare professionals to ensure seamless patient care.

• Accurately complete clinical documentation in a timely manner.

• Maintain compliance with agency policies HIPAA regulations Medicare Conditions of Participation and professional nursing standards.

• Deliver compassionate patient-centered care that aligns with Elara Carings mission and values.

• Current unrestricted Registered Nurse (RN) license in the state of practice.

• Minimum of one (1) year of nursing experience in an acute care skilled nursing or clinical care setting.

• Previous home health experience is preferre:d but not required.

• Experience with HomeCare HomeBase (HCHB) is a plus.

• Strong assessment communication and critical thinking skills.

• Reliable transportation valid drivers license and current automobile insurance.

• Ability and willingness to travel throughout the assigned service area.

• Physical Requirements- Sit stand bend lift and move intermittently and be able to lift 50100 lbs.
