Join Elara Carings Home Health team in the Attleboro area and help patients regain strength mobility and confidence in the place they feel most comfortabletheir own homes. Youll provide personalized care manage your daily schedule in the field and collaborate with a supportive clinical team.

As a Home Health Physical Therapist you will:

• Evaluate patients strength balance mobility and functional abilities
• Create personalized treatment plans based on each patients needs and goals
• Provide therapy that helps patients move safely and remain independent at home
• Teach patients and caregivers about exercises equipment and fall prevention
• Monitor progress and adjust treatment plans as needed
• Partner with physicians nurses therapists and other members of the care team
• Complete accurate and timely clinical documentation
• Provide guidance and clinical support to therapy staff when appropriate

• Degree from an American Physical Therapy Association-approved program
• Current unrestricted Physical Therapist license in the state of practice
• Valid drivers license dependable transportation and current auto insurance
• Ability and willingness to travel throughout Attleboro and surrounding communities
• Strong communication organization and independent decision-making skills
• Sit stand bend lift and move intermittently and be able to lift 50100 lbs.

Bring high-quality physical therapy directly to patients throughout the Attleboro area while building a career with flexibility support and purpose.
