Home Health Physical Therapist PT

Essential Duties and Responsibilities:

• Develop implement and adjust a conditioning/rehabilitation plan of care in partnership with patients representatives caregivers and physicians

• Test and screen patients physical strength to assist physicians in evaluating their level of function

• Educate and instruct patients family members or representatives about exercise programs and therapeutic procedures

• Accurately document observations interventions and evaluations pertains to patient care management and services provided

• Provide direct patient care as prescribed including evaluation and treatment planning

• Review physicians referrals and patients medical records to determine diagnosis and physical therapy treatment required

Minimum Qualifications (Knowledge Skills and Abilities):

• Graduation from an approved Physical Therapy curriculum

• Current and valid Connecticut Physical Therapist license

• Recent work experience in home health (preferred)

• Drivers License Car Insurance and Car Registration

• Strong clinical assessment organizational and communication skills

• Ability to handle multiple assignments simultaneously

• Ability to utilize computers and other electronic devices for tasks such as timekeeping in-servicing and documentation

• Bilingual (Spanish) preferred

• Strong interpersonal skills and good judgment

• Ability to work well under pressure and stay calm and focused in emergent situations

• Be highly self-motivated and results-driven

• A desire for further education to keep up with the ever-changing medical advances

• The work environment is typically a patients home with occasional visits to medical facilities or our office.

Salary:

• $90000 - $120000 ANNUALLY
