Location:Portland OR - Hybrid. This is a field-based home health position primarily serving patients in Gresham and surrounding communities.

Note:Candidates must have an active Oregon Physical Therapist (PT) license. Home health experience 1 year of Physical Therapist experience a Masters or Doctorate degree in Physical Therapy BLS/CPR certification a valid drivers license and reliable transportation HCHB/Homecare Homebase experience and OASIS experience are preferred.

Job Description

• Evaluate patients in their homes and assess mobility strength balance coordination pain and functional limitations.
• Develop individualized physical therapy treatment plans based on each patients needs and goals.
• Provide therapeutic exercises and techniques designed to improve mobility strength function and independence.
• Educate patients and caregivers on exercises safety precautions and self-care strategies.
• Monitor patient progress and adjust treatment plans as appropriate.
• Collaborate with nurses physicians therapists caregivers and other members of the interdisciplinary team.
• Complete accurate and timely clinical documentation.
• Travel throughout Gresham and surrounding communities to provide high-quality in-home care.

Qualifications

• Active Oregon Physical Therapist (PT) license required.
• Home health Physical Therapist experience preferred.
• 1 year of Physical Therapist experience preferred.
• Masters or Doctorate degree in Physical Therapy preferred.
• Current BLS or hands-on CPR certification preferred.
• Valid drivers license and reliable insured vehicle preferred.
• HCHB/Homecare Homebase experience preferred.
• OASIS experience preferred.
