Job Description:
Full-Time MondayFriday 8 a.m.5 p.m.
$38$42/hour based on experience No Weekends or On-Call
A great nursing visit can change someones entire dayfrom recognizing a concern early to helping a caregiver feel more confident or supporting a patient as they regain independence.
At Elara Caring were seeking a Licensed Practical Nurse to deliver compassionate skilled care in patients homes. Youll build meaningful relationships through one-on-one visits while working closely with an RN Case Manager and a supportive clinical team.

- Provide skilled nursing care according to each patients individualized care plan and physician orders.
- Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
- Teach patients and caregivers techniques that support health safety and independence at home.
- Collaborate with the RN Case Manager as patient needs and care plans change.
- Document visits nursing interventions and medication information accurately and promptly.
- Communicate with physicians patients families and the clinical team to keep care coordinated.
- Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.
- An active Massachusetts LPN license
- Graduation from an accredited nursing program
- At least one year of clinical nursing experience
- A valid drivers license reliable transportation and current auto insurance
- Ability to travel between patient homes and assigned work locations
- Ability to sit stand bend lift and move intermittently including lifting 50100 lbs.
