Provide one on one nursing care where patients feel most comfortable: at home. As a Home Health LPN with Interim HealthCare youll work alongside an RN and other care team members to support patients recovering from illness or injury or managing chronic conditions.

As a Home Health Licensed Practical Nurse heres a big-picture view of what youll do:
- Provide home-based care to patients with illnesses injuries and chronic diseasesoften after hospital dischargeunder the supervision of anRN
- Work with a team of physicians RNs CNAs and aides to deliver quality patient care
- Take patient vitals do assessments document progress and report changes to RN supervisor
- Assist RN in carrying out patients plan of care and teaching patient self-caretechniques
- Educate family on the patients plan of care medication and home safety
- Collaborate with interdisciplinary team to coordinate patient care and plan for discharge
A few must-haves for Home Health Licensed Practical Nurses:
- Graduate of an accredited practical nursing program and active LPN license inColorado
- Minimum of six (6) monthsof LPN experience ideally in home healthcare
- Wound and/or Med Surg experience is preferred
- CPR Certification (demonstration course required)
- Knowledge of state and federal home healthregulations
- Clinical proficiency in nursing strong communication skills and compassion
