Metro is looking for a motivated and passionate per diem Physical Therapist Assistant to provide in-home care under the supervision and direction of a licensed Physical Therapist. Job Type: Per Diem

  • Pay: $45-$55 per visit
  • Location: In and around Valley Stream

Responsibilities:

  • Accept and treat patients within your designated geographic area 
  • Provide physical therapy treatments to patients in their homes as outlined in the licensed PTs plan of care
  • Monitor and report changes in patient status to the supervising PT in a timely manner
  • Communicate effectively with patients caregivers and interdisciplinary team members to ensure coordinated effective care
  • Instruct patients family members caregivers and Home Health Aides in exercises safety awareness and use of assistive devices as directed by the supervising PT
  • Assist the PT in updating care plans based on patient progress and response to treatment
  • Document patient treatments and observations accurately and promptly in accordance with agency and regulatory standards
  • Participate in case conferences and contribute to the care planning and discharge process in collaboration with the PT
  • Adhere to all federal state and local regulations as well as agency policies and professional standards of practice

Qualifications :

  • Physical Therapist Assistant Program Graduate
  • Licensed Physical Therapist Assistant in the state of New York or eligible.
  • Mandatory Certifications: First Aid AED & CPR Certifications.
  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
  • Ability to travel to patients homes within the assigned geographic territory
