What Youll Do:
As a Health Services Coordinator (LPN) youll play a hands-on role in improving the lives of our residents by:
- Conducting comprehensive resident assessments prior to move-in re-entry and on an ongoing basis to ensure all physical mental and social needs are met.
- Assisting with community readiness for regulatory inspections including maintaining accurate service plans medication records and emergency preparedness.
- Establishing and fostering positive relationships with residents families associates home health hospice and other external partners.
- Coordinating and ensuring proper health-related training and education for the caregiver team conducting skill observations to optimize resident care delivery.
- Providing direct resident assistance as needed including personal care activities and administering medication in accordance with physician orders.
What Were Looking For:
- An active Nursing license with the state.
- A current CPR/BLS certification.
- 1 years of experience in a senior living or related industry supervisory or leadership role.
- 1 years of experience providing care to cognitive and memory-care residents or patients (preferred).
- Valid drivers license and acceptable driving record; ability to provide resident transportation and/or serve alcoholic beverages at community events.
- The ability to assist residents with limited mobility lift/push/pull up to 50lbs.
- Excellent verbal and written communication skills with the ability to effectively collaborate with diverse groups navigate challenging situations and make sound decisions.
- Strong technical skills (e.g. Word Excel PowerPoint) and the ability to maintain confidential and privileged information (including HIPAA residents and associates).
