Position Summary (Purpose of Job):

The Health & Community Navigator Supervisor (M2) provides day-to-day leadership for Health & Community Navigators supporting employees across Distribution Centershelping ensureconsistent executionstrong field support rolesupportsthe team through changingsiteprioritiesresolvingday-to-dayoperational issues andusesfield insightsto recommend practicalimprovements that strengthen care delivery associate engagement andworkflowefficiency.

The supervisorusesdatareporting toolsandavailable technologyto streamline workflows improve reporting accuracyidentifytrends andsupport consistent repeatableprocesses.

Thisindividualmusthavedatasavviness with the ability to interpret performance resultsand recommend changes that improve supervisor manages program-related travel supplies and expenses within approved budget ranges monitorsspend andsupports responsibleresourceuse.

The supervisordemonstratesprofessionalpresence andclearcommunication skills while partnering with site leaders HRinternal partners and community partners. The role is expected to be in the field with the team modeling best practicesidentifyingimprovement tactics responding tosite-leveloperationalneeds andsupportingprogram value efficiency and results.

  • Provide direct leadershipand accountability for Health & Community
  • Navigators tosupportconsistent execution associate engagement andteam performance.

  • Coach Navigators through regular 1:1s field observation onboarding and real-time feedback to improve consistency confidence and impact.
  • Lead team meetings knowledge sharing and professional development activities that translate lessons learned into repeatable best practices.
  • Use data associate feedback and site-level insights toidentifyopportunities close performance gaps and strengthen service delivery.
  • Support career development engagement and retention by building capability reinforcing accountability and recognizing high-impact performance.
  • Address performance concerns throughtimelycoaching development planning and corrective action processes when applicable.
  • SupportProgram Operations Efficiency andResource Use(40%)

Manage day-to-day program execution resolve operational issues and usedata available technology and process improvement tosupportefficiency care delivery impact andconsistent program performance.

  • Interpret data andrecommend practical next steps to supportcare navigation workflow efficiency associate experience andconsistent service delivery.

  • Resolve daily operational issues staffing changes workload shifts and urgent business needs whilemaintainingservice continuity and quality.
  • Review documentation activity tracking and reporting for accuracy consistency compliance and readiness formanager or stakeholderreview.
  • Manage program-related travel supplies and expenses within approved budget ranges to supportresponsible resource use andoperational discipline.
  • Prepare concise operational updates and performance insights thatsummarizeimpactresource use risks and recommended actions formanager or leader review.

PartnerwithStakeholdersto SupportProgramGoals(10%)

Partner with Distribution Center leadership HRinternal partners and community partners tounderstandsite needssupportoperational priorities andstrengthenprogramexecution.

  • Usestakeholder feedback and field observationsto supportpractical improvementsin engagementandconsistent service delivery.
  • Coordinate awareness campaigns education events and engagement activities that support associate access health literacy and resource navigation.
  • Surface risks barriers and opportunities early andshare recommendations for manager or leader review.

Lead in the Field and Improve Site Execution (30%)

Work directly with assigned sites and Navigators to model best practices troubleshoot barriers strengthen associate engagement andsupport consistentservice delivery.

  • Provide direct field support to one or more Distribution Centers whilemaintainingvisibility into frontline needs associate barriers and site-specific opportunities.
  • Model high-quality associate engagement follow-up health literacy education benefits navigation and community resource connection practices.
  • Partner with associates toidentifybarriers connect them toappropriate resources and document outcomes thatsupport visibility into associate support and service impact.

  • Observe field executionidentifyimprovementopportunities andsupportrecommended adjustmentsthat strengthen consistency efficiency and associate experience.

Minimum Job Qualifications (Knowledge Skills & Abilities):

  • Education:Associates degree or higher preferred; equivalent experience in operations community health employee support or field-based program delivery may be considered.
  • Experience: 6 years of relevant experience with 13 years ofpeoplesleadership field operations program implementation or team coaching experience.

Leadership:Demonstratedability to lead teams manage priorities resolve operational issues coach performance andreinforceaccountability in a changing environment.

  • Data and technology:Data savvy with the ability to interpret results use reporting toolssupport technology-enabled workflows and translate insights intopractical actions.
  • Communication:Strong written and verbal communication skills withprofessionalpresence and the ability to explainprogram updates operationalimpact risksresource needsandrecommendationsto stakeholders.
  • Preferred:CHW certification; experience withSDoH care navigation employee engagement Distribution Center operations or field-based teams.
  • Working Conditions:

Hybrid role requiring onsite presence at Distribution Centers and virtual work. Travel estimated at2035%.

Physical Requirements:

Ability to perform computer-based work andparticipatein onsite engagement activities in Distribution Center environments.
