Utilize Microsoft Windows programs to track and maintain data for multiple rolling deadlines related youth in careCommunicate with external team partners including DCPP CMO and family members of youth Conduct home visits and office base services and accompany individuals being served as needed Responsible for the annual contractual review process for Host Home ProvidersMaintain and audit files regularly for complianceCoordinate care with team members to ensure youth receive high-quality care Exceptional organization and time management practices requiredFlexible in daily schedule to best facilitate program needs Interface effectively with internal team members to ensure licensing regulations are metPerforms other related duties and activities as requiredQualifications: Bachelors degree in human services. .Three years of related work experience and knowledge of case management. Valid drivers license registration and insurance. Ability to establish working relationships with individuals served.
