Essential Job Functions
Provide admissions coverage at Integritus Healthcare facilities in Bourne Yarmouth and New Bedford with assignments based on operational needs.
Coordinate the resident admission process from initial referral through admission.
Collect and review clinical and financial information confirm payer sources and obtain required approvals.
Schedule and conduct tours and communicate with prospective residents families and referral sources.
Complete admission paperwork and ensure OBRA PASRR and other admission requirements are satisfied.
Coordinate new admissions with nursing rehabilitation social services dietary the Business Office and other departments.
Maintain accurate referral and admission records and provide timely updates to facility leadership.
Support census goals customer service referral relationships and facility marketing efforts.
Identify and communicate admission barriers or process concerns and work with facility leadership to resolve them.
Follow all confidentiality resident rights safety infection control and company requirements.
Qualifications
High school diploma or equivalent required; bachelors degree preferred.
At least three years of long-term care or related healthcare experience preferred.
Admissions referral management or skilled nursing experience strongly preferred.
Working knowledge of Medicare Medicaid OBRA PASRR and long-term care admission requirements.
Strong organizational communication customer service and problem-solving skills.
Ability to work independently manage competing priorities and collaborate with interdisciplinary teams.
Proficiency with basic computer applications and referral-tracking systems.
Valid drivers license reliable transportation and the ability to travel regularly between Bourne Yarmouth and New Bedford.
Flexibility to change facility assignments based on business needs.
