Position Summary
The New Jewish Homes Certified Home Health Agency (CHHA) is seeking dedicated and experienced Per Diem Speech-Language Pathologists (SLPs) to join our Community Services team.
In this role the Speech-Language Pathologist will provide home-based speech therapy services to adult and geriatric patients throughout Manhattan. Responsibilities include conducting comprehensive speech language cognitive-communication and swallowing assessments developing individualized treatment plans and providing therapeutic interventions designed to maximize patients communication abilities swallowing function safety and overall quality of life.
The ideal candidate will be committed to delivering high-quality patient-centered care while collaborating effectively with an interdisciplinary healthcare team.
- Conduct speech-language and swallowing screenings evaluations and assessments as prescribed by the physician.
- Develop and implement individualized treatment plans to address speech language voice cognitive-communication and swallowing disorders.
- Provide therapeutic interventions and rehabilitative services based on each patients clinical needs and functional goals.
- Educate patients and family members regarding treatment plans progress home exercise programs and discharge recommendations.
- Prepare and maintain accurate clinical documentation including evaluations progress notes treatment records and discharge summaries.
- Monitor patient progress and modify treatment plans as appropriate.
- Collaborate with physicians nurses therapists social workers and other members of the interdisciplinary care team to ensure coordinated patient-centered care.
- Participate in case conferences team meetings and care planning discussions.
- Maintain required departmental and regulatory records in a timely and accurate manner.
- Assist in maintaining therapy equipment supplies and work areas.
- Participate in professional development and educational activities to remain current with best practices and advancements in the field.
- Perform other duties as assigned.
- Current unrestricted New York State license as a Speech-Language Pathologist.
- Experience working with a geriatric population.
- Availability to provide home visits throughout the Manhattan service area.
- Strong assessment clinical reasoning communication and documentation skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Masters Degree in Speech-Language Pathology or a related field.
- Previous home care or community-based healthcare experience.
- Experience treating adult and geriatric patients with speech language cognitive and swallowing disorders.
- Strong clinical assessment and treatment skills.
- Excellent verbal and written communication abilities.
- Ability to establish effective therapeutic relationships with patients and families.
- Strong organizational and time-management skills.
- Commitment to delivering compassionate patient-centered care.
- Ability to work autonomously in a home care setting.
