Job Summary
We are looking for a full-time Family Service Coordinator to join our South Bend IN team!

Compensation starting at $50000.00
Hours: Monday - Friday 9AM-5PM with flexibility needed for evening and weekend hours as well.
PositionSummary
Under the general supervision of the Clinical Supervisor or designee this position works to make a meaningful difference in the lives of the children and families served. This position works closely with youth family foster parents and community partnersproviding advocacyand support toward the ultimate goal of living safely and successfully in the community.

Responsibilities
- Activelysupportsrepresentsandextendsthemissionvisionandvaluesoftheorganization.
- Provides in home supportive services transportation parenting education counseling community resource referral advocacy and other social service interventions to the youth and family meeting or exceeding established outcomes thresholds on behalf of persons served.
- Travels daily to provide community based services to and on behalf of youth and families in compliance with organizational contract and regulatory requirements.
- Completesallrequireddocumentationinatimelymanner.
- Completesallrequiredtrainingsinatimelymanner.
- Coordinates and monitors services for the youth and family in the community including but not limited to mental health medical educational psychological vocational and social services.
- Regularlyreviewsandmodifiesyouthbasedplanstoassureconsistentprogressandsuccessonwrittenplangoals.
- Establishesandmaintainsstrongrelationshipwithassignedfosterparentsandfamiliesoforigin.
- Assists in family reunification and/or stabilization efforts with the youths primary family or other permanent living arrangement consistent with the youths case plan.
- Providessafetyassessmentcrisisresponseandbehavioralstabilizationservicesasrequiredtoassuresafetyandstabilityforyouthserved.
- Consistentlyachievesestablishedproductivitythresholds.
- ParticipatesintheContinuousQualityImprovementactivitiesonamonthly/quarterlybasis.
- Activelyparticipatesintheweeklysupervisionprocess.
Minimum Qualifications
- Bachelors DegreeinSocialWorkorcomparableHumanServicesfieldfromanaccreditedinstitution.
- 2yearsofworkexperienceworkingindirectservicewithyouthandfamiliesstronglypreferred.
- Must have flexibleavailability as needed.
- Musthaveavaliddriverslicensereliabletransportationautomotiveinsuranceandagooddrivingrecord.
- Must have car insurance requirements of 100000/300000 bodily injury insurance.
- Awillingnesstoworkflexibleandnon-traditionalhoursintheserviceoffamiliesoforiginandpersonsserved.
- Capacitytobeflexibleandresponsivetoyouthservedfostercaregiverssystempartnersandinternalcustomers.
