Job Summary
Project HOME is seeking a Family Nurse Practitioner to join our integrated healthcare team and help eliminate health disparities for individuals and families experiencing homelessness housing insecurity and systemic barriers to care. As a Family Nurse Practitioner you will deliver primary care services to children adults and OB/GYN patients.
The hours are Monday-Friday 8:00 AM-4:00 PM located at the Stephen Klein Wellness Center 2144 Cecil B. Moore Ave Philadelphia PA 19121.
What Youll Do:
As a key member of our community-based care team youll:
- See 9 patients per session 8 sessions per week with a focus on holistic and trauma-informed care
- Collaborate with a dedicated team of medical assistants behavioral health consultants care managers and peer specialists
- Address both clinical and social determinants of health recognizing the unique needs of marginalized populations
- Uphold our mission and values through ethical equitable and evidence-based care
- Participate in quality improvement initiatives staff development and clinical leadership opportunities
- Mentor students (NP PA and MD) and engage in ongoing CME for professional growth

What Were Looking For
- Masters Degree and current NP license (Family Nurse Practitioner required) with three (3) years of practice experience serving gynecology obstetrics and pediatric patients
- CPR and DEA certification
- Experience or strong interest in serving underserved and vulnerable populations
- Comfort working with patients experiencing trauma behavioral health concerns or housing insecurity
- Strong communication clinical and organizational skills
- Experience with eClinicalWorks or similar EHR systems preferred
- Light lifting standing and walking throughout the day as needed
