What you bring to SBHG (qualifications):

Education:

  • Facilitator II: Masters degree in Mental Health Human Services or closely related field
  • Facilitator I: Bachelors degree in Mental Health Human Services or closely related field

Experience

  • Facilitator II: One (1) year of experience in a mental health field and direct service experience with severely emotionally disturbed children adolescents and families; experience with high-risk children and families.
  • Facilitator I: Two (2) years of experience in a mental health field and direct service experience with severely emotionally disturbed children adolescents and families; experience with high-risk children and families.

License or Certification

  • Valid California Drivers License and maintenance of a driving record that meets the company driver eligibility policy
  • CANS Certification within 30 days of hire
  • Completion of Statewide Standardized Child and Family Team (CFT) Facilitation Training within one (1) year of hire

How you will make a difference (job overview):

The Facilitator serves as the care coordinator and central guide of the Wraparound process. Facilitators coordinate lead and monitor implementation of the individualized Plan of Care ensuring the process reflects the familys voice culture strengths and goals. The Facilitator is ultimately the key to providing high fidelity Wraparound ensuring each phase follows best practice and meets the Wraparound Standards. Facilitators implement high fidelity during the four Wraparound phases of Engagement Plan Development Implementation and Transition which includes upholding the ten Wraparound principles. They help unify team efforts maintain team cohesion and coordinate services across the Childrens System of Care including child welfare mental health juvenile justice education and tribal systems in the case of an American Indian child. Collaborates with team members to ensure adherence to program process and principles as well as philosophy of whatever it takes.

  • Facility & Setting:Community home-based and juvenile justice systems (Ranch juvenile hall)
  • Clinical Services:Provides mental health services to youth involved in juvenile and child welfare systems
  • Skills & Services:Wraparound approach provides services to entire in 24/7 on-call system; pay differential.
