POSITION SUMMARY The Engagement Specialist provides centralized engagement outreach and care coordination support to children and adults receiving behavioral health services. This role is responsible for improving patient access retention and continuity of care through proactive communication follow-up and coordination with clinical teams and referral position operates from a centralized location alongside the Care Management team supporting multiple programs health center sites and service AND EXPERIENCE/QUALIFICATIONS High School diploma required.
