(CBI) Discharge Supervisor is responsible for overseeing all aspects of discharge for patients receiving services in Crisis Inpatient or Transition Point programs. The Discharge Supervisor is actively involved in patient discharge planning from time of admission and ensures that family members enrolled providers and other identified supports are actively engaged and involved in the process. The Discharge Supervisor attends 4-hour staffings and daily huddles with the multidisciplinary team to identify any potential discharge barriers and provide feedback on current discharge plan. The Discharge Supervisor ensures the discharge transition plan accurately reflects the discharge plan including placement name address outgoing referral and follow up appointments. The Discharge Supervisor works directly with the Clinical Manager Clinical Director and Operations team to ensure that discharge planning and services are effective and efficient in addressing patient/participant and community needs as well as are delivered in accordance with CBI Clinical Policy and Procedure Arizona Administrative Code ADHS/DBHS Provider Manual Code of Federal Regulations and contract and Experience RequiredBachelors Degree in related field required Masters Degree preferred.1-3 years of full-time Behavioral Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.
