Discharge Planner (Social Worker)

In this role youll build trusting relationships with residents and families helping them navigate important transitions and providing support when it matters most. Youll work alongside a caring interdisciplinary team to advocate for resident needs coordinate care and ensure each individual receives the resources and guidance needed to thrive.

What Youll Do:

• Assist with resident assessments care planning and discharge planning.
• Conduct cognitive and mood evaluations and coordinate follow-up services as needed.
• Provide emotional support guidance and education to residents and families.
• Connect residents with community resources financial assistance programs and social service agencies.
• Maintain timely documentation and collaborate with the care team to support resident well-being.

What Youll Bring:

• Licensed Social Worker (LSW) in the State of Michigan.
• Bachelors or Masters degree in Social Work or a related human services field.
• Healthcare social work experience preferred.
• Experience working with older adults or in long-term care preferred.
• A compassionate resident-centered approach and strong communication skills.
