Responsibilities

The Direct Support Professional provides individualized support to people with developmental disabilities in a self-directed setting. This role helps individuals build and maintain daily living social and adaptive skills while working toward personal goals greater independence community integration and an enhanced quality of life.

• Provide individualized support in accordance with each persons service plan preferences needs and personal goals
• Promote independence and skill development by assisting individuals with daily living self-help socialization and adaptive skills
• Support personal and community-based goals that encourage independence productivity meaningful participation and community integration
• Provide safe transportation and First Aid/CPR support as requested by the Self-Directed participant
• Complete required documentation accurately and on time including EVV requirements monthly summaries mileage records reimbursement submissions and other required reports
• Maintain confidentiality professionalism and compliance with agency policies and procedures while attending required meetings and trainings

• High School Diploma or GED
• Valid New York State Drivers License
