JOB TITLE: Direct Support Professional In-Home Services
DEPARTMENT: Community Resources
SUPERVISORS TITLE: In-Home Services Regional Manager or In-Home Services Manager/SFC
POSITIONS SUPERVISED: None
JOB STATUS: Full-Time Non-Exempt and Part-Time Non-Exempt
SCHEDULE: Schedule varies; Overtime hours currently available (although not guaranteed)
Provide individualized in-home and community-based support to individuals with disabilities. This role focuses on promoting independence developing life skills and supporting individuals in achieving personal goals while remaining in their home environment.
Assess individual needs and assist in developing and implementing individualized service plans
Provide training in daily living skills including personal care communication and social development
Support individuals in accessing community resources and participating in community activities
Assist with household tasks organization and maintaining a safe home environment
Escort individuals to medical appointments and community activities as needed
Support individuals with correspondence budgeting and bill paying when necessary
Maintain accurate documentation of services progress and outcomes in compliance with regulations
Develop materials and maintain supplies to support individual goals
Act as a resource to families caregivers and other team members
Promote independence self-advocacy and personal growth
Maintain positive relationships with individuals families and community partners
Remain flexible to meet the needs of individuals served
Strong interpersonal and communication skills
Ability to work independently in home and community settings
Empathy and ability to support individuals with diverse needs
Strong organizational and documentation skills
Ability to collaborate effectively with team members and families
Basic computer skills (word processing spreadsheets and data entry)
Ability to lift and carry up to 50 pounds
Frequent bending stooping and assisting individuals with mobility
Ability to travel between client homes and community locations
Valid drivers license reliable transportation and proof of insurance required
CPR and First Aid certification
Preventing Disease Transmission (PDT)
Certified Restraint Training (CRT)
Medication Administration (Core A & B)
Completion of required trainings
