What Youll Do (Essential Job Functions)
Follow individual care plans and report any concerns to nursing or management.
Administer medications and treatments with proper documentation per policy.
Support daily living needs including:
Personal care
Meal preparation and dietary support
Household tasks and errands
Health appointments and community integration
Building natural supports and social relationships
Promote health safety and a clean home environment.
Remain calm and effective during challenging situations.
Foster a positive respectful team dynamic with coworkers individuals and their families.
Accurately document individual progress and changes.
Attend all required trainings meetings and ongoing professional development.
What Youll Need (Minimum Requirements)
High school diploma or GED
Valid drivers license and reliable transportation
Ability to lift 25-50 lbs. (with assistance or lifting devices)
Physical ability to bend squat push/pull and kneel as needed
