BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) to join our Hudson County Visits team.

This position provides one-on-one home health visits for adult and geriatric clients throughout Hudson County NJ with an immediate opening in the southern Hudson Countyarea.

• Perform skilled physical therapy visits throughout designated territories.
• Complete comprehensive physical therapy evaluations and ongoing reassessments.
• Assess mobility strength balance gait transfers endurance and overall functional status.
• Develop and implement individualized treatment plans using evidence-based interventions.
• Provide therapeutic exercise gait training balance training manual therapy techniques and patient education.
• Collaborate with nurses physicians and interdisciplinary team members to coordinate exceptional patient care.
• Educate patients and caregivers on exercises safety strategies fall prevention and disease management.
• Complete timely and accurate documentation using BAYADAs electronic medical record (EMR) system.

• Current New Jersey Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Home health experience preferred but not required.
• Strong communication organizational and clinical assessment skills.
• Ability to work independently and effectively manage a flexible schedule.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel throughout the assigned territory.

Base Pay depending upon qualifications status and subject to negotiation:

$77.00-$83.00 per point ($120120-$129480 annual earning potential)

$10000 Sign on Bonus! for full time PT positions.
