Job Summary

Market Quality CoordinatorJob Summary: The Market Quality Coordinator is responsible for the monitoring and pursuit of CMS STARS HEDIS measures conducting patient outreach calls and partnering with center leadership and teams to achieve STARS and HEDIS goals. This role serves as part of the market operations team working across many payors within a single market in an outpatient senior-focused primary care enterprise and local strategies and initiatives for closing STARS gapsMedical record review to document and close Focus On Care measuresRequest medical records and upload documentation to electronic medical records systemPerform patient outreach calls to support quality gap closure including assisting and encouraging members with scheduling and completing and perform ancillary visits to complete in-center regularly with Market Leaders to discuss roadblocks regular progress and to strategize center and market focused compliance with OSHA safety standards and conduct accurate legal and ethical documentation at all responsibilities and duties as Skills/AbilitiesWorking knowledge of quality measures and quality gap closure working in a primary care with Microsoft Office Suite products including Word Excel Power Point Outlook and TeamsDemonstrated team building to work independently with a high level of attention to to the needs of culturally diverse staff patients and HIPAA standards and confidentiality of protected health information; reports critical incidents and serves as a role model of ArchWell Healths Values:Be compassionateStrive for excellenceEarn trustStay resilientAlways do the right thingEducation and ExperienceHigh school diploma or equivalent education (GED) required.A minimum of 1 year work experience as a Medical AssistantGraduation from a nationally accredited Medical Assistant program OR comparable healthcare vocational training (i.e foreign-trained physician paramedic etc.) requiredBLS for Healthcare Providers requiredEClinicalWorks or similar EMR experienceExperience working with geriatric patients is a plusExperience with the CMS STARS program (HEDIS) preferredEmbodies and serves as a role model of ArchWell Healths Values:Be compassionateStrive for excellenceEarn trustShow respectStay resilientAlways do the right thingAbout ArchWell Health:At ArchWell Health were creating a community of caring designed to help our members stay healthy and engaged.
