We are seeking a Community Transition Liaison Supervisor provides general oversight and supervision of CTLP
activities.

What you will do:

Serves as the primary contact for the Community Transition Liaison Program (CTLP) to AGE
Ensures the quality of the overall administration of the CTLP in accordance with programmatic
requirements set forth by AGE
Provides supervision direction and oversight to the CTLP Team(s)
Ensures timeliness and quality of all CTLP documentation including consumer and service data
in Aging & Disability (A&D)
Monitors case records in compliance with regulations standards and protocols
Contributes to the development of procedures standards and guidelines for the operation of
the team
Collaborates with facility staff CTLP staff and other OCES staff as appropriate to plan and
coordinate discharge activities
Maintains a detailed knowledge of current OCES policies and standards and funding source
regulations
Other duties as assigned.

What you will bring:

Bachelors degree in social work Nursing Human Services or related field
Current Social Work or Registered Nurse license
Three years of supervisory or managerial experience and a minimum of one year of transition
support experience Excellent verbal and written communication skills.
Excellent interpersonal and conflict resolution skills.
Excellent organizational skills and attention to detail.
Strong analytical and problem-solving skills.
Strong supervisory and leadership skills.
Proficient with Microsoft Office Suite or related software
