Community Support Specialist Residential
Job Summary

- Deliver individualized strengths-based services using a motivational stage-matched approach
- Partner with individuals to identify goals and develop and update Wellness Plans
- Provide holistic support across the Eight Dimensions of Wellness including physical and mental health
- Coach and build skills in daily living communication employment/education and independent living
- Support individuals in accessing community resources such as housing financial assistance and healthcare
- Collaborate with integrated care teams and natural supports to ensure coordinated whole-person care
- Assist with care transitions including hospital discharge planning and follow-up support
- Accompany individuals to appointments and provide ongoing engagement and advocacy
- Maintain timely accurate documentation and meet service expectations
- Provide transportation as clinically indicated
- High School/GED required
- Associates/Bachelors degree in a human service-related field preferred
Must qualify for position based on overall CSS requirements in at least one of the options listed:
- Qualified addiction professional;
- Bachelors degree in a human services field from a college or university included in the U.S. Department of Educations database of accredited schools at http:/ ;
- Any four-year degree or combination of higher education and qualifying experience;
- Four years of qualifying experience; or
- Associate of Applied Science in Behavioral Health Support degree as designated by the department.
- Qualifying experience must include delivery of service to individuals with mental illness substance use disorders or developmental disabilities.
- A valid unrestricted drivers license and agency established minimum automobile coverage required
- Lead with empathy and believe in a recovery-oriented person-centered approach
- Naturally focus on strengths and possibilities rather than limitations
- Enjoy working both independently and as part of a collaborative care team
- Are adaptable and comfortable working in residential and community-based settings
- Communicate effectively with diverse individuals families and providers
- Are organized and able to balance documentation with meaningful client engagement
