Position Summary
The Community Support Specialist is responsible for providing person-centered support and care plan execution during overnight hours. This role ensures client safety and well-being through daily welfare checks observation of behavioral changes and engagement techniques. The Community Support Specialist supports client progress toward housing stability and health outcomes by executing care plans maintaining a safe and stable environment and completing accurate documentation to ensure continuity of care.
The ideal candidate is dependable attentive calm under pressure and able to remain alert throughout the overnight shift.
Key Responsibilities
- Conduct regular welfare checks and check-ins to monitor the safety and well-being of all clients.
- Observe and document changes in client engagement health status functioning or living environment promptly escalating concerns to the Care Team.
- Execute assigned care plan activities including practical assistance and system navigation support ensuring measurable progress toward client goals.
- Utilize motivational interviewing and verbal de-escalation techniques to support clients and address behavioral substance use or medical concerns.
- Maintain accurate timely and objective documentation of all client encounters observations and care plan activities within organizational standards.
- Support a clean safe and orderly living environment by observing facilities for health and safety concerns and addressing them in accordance with procedures.
- Participate in shift handoffs communicating significant client updates (changes in engagement barriers or safety risks) to the incoming Care Team.
- Uphold confidentiality maintain appropriate professional boundaries and comply with all regulatory and organizational policies.
- Participate in required trainings including Mental Health First Aid Trauma-Informed Care and Behavioral Crisis Recognition.
Qualifications
- High School Diploma or GED required.
- Previous experience in a residential care or advocacy setting preferred.
- Ability to remain awake and alert throughout the shift.
- Strong observation communication and problem-solving skills with a client-centered strengths-based approach.
- Proficiency with computer systems (Google Workspace) and Electronic Health Records (EHR).
- CPR/First Aid certification (or willingness to obtain upon hire).
- Background check and drug screening required.
Physical Requirements
- Ability to remain on your feet or seated for extended periods.
- Capable of responding quickly to emergency situations or escalating client needs.
- May involve light housekeeping organizing or lifting up to 25 lbs.

Work Environment
This position operates in a fast-paced mission-driven healthcare and housing services environment supporting staff who work directly with vulnerable populations. This position works primarily during overnight hours in a supportive housing or group-living environment. The Community Support Specialist (Graveyard) will interact with residents who may have physical mental health behavioral or other support needs and must be able to respond with professionalism sound judgment respect and appropriate boundaries.
